CASE MANAGEMENT AND COMMUNITY PSYCHIATRIC SUPPORTIVE TREATMENT AS WELL AS HEALTH HOMES IS AVAILABLE AT THE FOLLOWING LOCATIONS:     WARREN     PROVIDENCE     NEWPORT


All are conveniently located close to a bus line.


WARREN AND PROVIDENCE: Both serve adults with severe and persistent mental illness (SPMI) who may also have co-occurring substance abuse issues. Both provide services 24/7 based on the PACT (Program of Assertive Community Care) model and draw on a multidisciplinary team that includes Psychiatrists, Registered nurses, therapists, substance abuse specialists, employment specialists, case managers, and a consulting Clinical Pharmacist. The Warren team was established in 1986 as the first Mobile Treatment Team in the state of Rhode Island and has maintained the philosophy of working with the whole individual as the best success for recovery. Providence is unique in that it provides outreach services and works with individuals who were formerly homeless.


Both are licensed by the RI Department of Behavioral Health, Developmental Disabilities, and Hospitals (BHDDH) for Case Management, Community Psychiatric Supportive Treatment, and Medication Services and certified by Medicaid as a Health Home provider. They are CARF accredited in Case Management/Services Coordination: Mental Health (Adults).


Each site provides intensive integrated treatment, rehabilitation and support in the community to help meet the person's complex needs. The individual is provided with services that support successful independent living in the community. For example, staff may help a person solve a problem, manage finances, access resources, purchase groceries, attend a medical appointment, prepare for a job interview or find an apartment. The person may receive counseling, job counseling, substance abuse counseling and coordination of medical care. Assistance can include visits up to twice a day to make sure medication is taken on the proper schedule. Advocacy in both civil and criminal court cases is provided to those with legal issues.


NEWPORT: This is the newest site and provides CSP, housing, and Health Home services. Initially, an outgrowth of the Providence Housing First program, it augments staff and services with both Warren and Providence resources. It also is based on the PACT model and provides services to individuals who were formerly homeless as well as outreach services.
